Fixed Price Dental: Terms and Conditions

 

These Terms and Conditions apply to the following dental services provided by Fixed Price Dental approved Dentists

Services

We reserve the right to unilaterally amend these Terms and Conditions at any time. All amendments to these Terms and Conditions will be posted to the Fixed Price Dental website.

General

  • The fixed fee is for each standard fixed fee service or any part thereof.
  • If any of the included treatments for any particular services are not required the cost of the appointment will not be reduced.
  • The fixed fee will not be altered in relation to the number of teeth present in a patient.
  • Bitewing x-rays and Panoramic OPG (Full mouth) x-rays may not be required at every visit but may be recommended for all new patients as a baseline recording.
  • The patient’s (or parent/caregiver’s) consent will be sought before performing any additional treatment(s) beyond the initial Service. A separate appointment may be required to perform any additional treatment(s).
  • If the recommended additional treatment that is not a part of the Fixed Price Dental scope of Services the patient will be referred to a dental practitioner who is suitably qualified to perform that treatment . That refereed dental practitioner will inform the patient of any fees which may be associated with that additional treatment.
  • For the avoidance of doubt, Fixed Price Dental is not liable for any loss or damage caused by any practitioner, the practitioner hold their own medical indemnity insurance and is liable for any work they have performed on patients.
  • For more information about whether private health fund rebates may apply to you, please consult your Private Health Insurer.
  • Bulk-billed dental items are subject to the terms and conditions applying to the child dental benefits schedule. Please refer to the Australian Government Website for further information.
  • Set out below are the treatments included within each of the Services.

 

Fees and Cancellation Policy

If you cancel a booking before you make booking payment through the online booking system (not follow through the complete online booking procedure) your booking will not be confirmed and therefore no further action is to be taken by yourself.

If you cancel or FTA (fail to attend) a confirmed booking which you have prepaid your booking fee online, you will forfeit your booking fee.

If you are 20 minutes late or more for your appointment, then this is treated as a cancellation and you will forfeit your pre-paid booking fee.

Payment Service

Our website includes a Payment Service. You must use the payment service to pay your treatment booking fee (this booking fee is deducted from the total treatment cost). The balance of the treatment cost (less the booking fee) is to be paid directly to the Fixed Dental approved dental clinic on the day of treatment. The payment service may be provided by one or more third parties  (the payment provider)

Your payment of the booking fee for each specified treatment will be taken to be your consent to receive that treatment. Your card details are processed and handled through the Payment Service. Your Payment details will be stored by the payment gateways and a secured token will be provided to Fixed Price Dental. The Payment Provider adheres to the standards set by the “Payment Card Industry Data Security Standard”.

 

Private Health Insurance

Patients with private health insurance that includes dental extras can use their private health insurance to make a claim on the dental treatments they receive from Fixed Price Dental approved Dentists

We have listed in our Terms and Conditions the Dental item codes for that can be claimed by the Fixed Price Dental Dentists from the private health funds. No other item codes can be claimed other than what is listed.

Certain private health funds will pay more than the advertised treatment cost on the Fixed Price Dental website, in this instance the approved Fixed Price Dental clinic will note this amount as a credit on the patients clinic file and the patient can use this credit towards any future treatment. In some cases the private health fund will pay less than the advertised fee on the Fixed Price Dental website, in this instance the patient will need to make up the shortfall or (gap payment). The gap payment will be the difference between the amount the private health fund pays (once the allowed dental item codes are claimed) and the advertised cost of the Fixed Price Dental treatment (less the booking fee that the patient has already paid)

*The exact amount that the private health fund pays on dental services depends on the level of the patients cover.

 

How We Provide the Services

We arrange for third party approved dental clinics to provide the treatments. If for any reason WE need to defer or cancel a booking, then we will provide you notice via email and text, you will be fully refunded your prepaid booking fee. If your booking is to be rescheduled for this reason, you will be responsible for making a new booking through our online booking system. We will not be liable for any claim arising from you having to reschedule a booking.

 

How We Will Communicate With You

We will communicate with you via email or text (SMS) message. By creating a booking, you agree that we may send you email and text messages (SMS) as part of the normal business operation of the services. You can also communicate with us through our Online chat facility (On our website).

 

 

Fixed Price Dental- Fixed Price Dental Treatments (T&C’S)

 

Check Up and Clean

Only the following treatments are included (as may be required)

  • Initial examination or checkup
  • 2 x Bitewing x-rays
  • 1 x Full mouth x-ray
  • Removal of calculus
  • Plaque/ stain removal
  • Topical application of remineralising and/or cariostatic agent

*The Fixed Price Dental checkup & clean treatment covers the following dental codes -examination (011, 012), scale and clean (114) or removal of plaque (111), fluoride (121), x-rays (022 x 2) and OPG (037) unless one or more of these items are deemed unsuitable for the patient, in the clinical opinion of the dentist. Fixed Priced approved Dentists must use only these codes for this treatment.

 
 

Fillings

Only the following treatments are included (as may be required):

  • 1 x Bitewing x-ray or 1 x periapical x-ray
  • 1 x Full mouth x-ray – if required
  • Diagnosis of the issue
  • Local anaesthetic to numb the tooth during the procedure.
  • A tooth-coloured direct restoration involving up to four surfaces of a single tooth (commonly known as a filling).
  • For the avoidance of doubt, teeth requiring a filling involving more than 4 tooth surfaces will not be included and the patient will be referred to another dental practitioner.

*The Fixed Price Dental filling treatment covers the following dental codes:  x-rays (022 x 2) and OPG (037), (531) – one surface filling OR (532) – two surface filling OR (533) – three surface filling or (534) – four surface filling , unless one or more of these items are deemed unsuitable for the patient, in the clinical opinion of the dentist. Fixed Priced approved Dentists must use only these codes for this treatment.

 

Pain Relief- emergency toothache

Only the following treatments are included (as may be required):

  • Limited examination (diagnosis)
  • 1 x Bitewing x-ray or 1 x periapical x-ray
  • 1 x Full mouth x-ray) – if required
  • Diagnosis of the cause of the pain
  • Local aesthetic to numb the tooth during the procedure

Depending on the cause of the pain one of the following options may also be included:

  1. First stage Root Canal Therapy
  2. Tooth Desensitising application
  3. Deep cleaning around a tooth to treat an acute gum infection
  4. A temporary filling  (for cases where this is the most appropriate solution to relieve pain until a more longterm solution can be performed)

*The Fixed Price Dental Pain relief- emergency appointment covers the following dental codes:  X-rays (022 x 2) and OPG (037), (419) Extirpation, (572) Temporary filling, unless one or more of these items are deemed unsuitable for the patient, in the clinical opinion of the dentist. Fixed Priced approved Dentists must use only these codes for this treatment.

 

Basic Tooth Extraction

Only the following treatments are included (as may be required):

  • 1 x periapical and 1 x Bitewing x-ray – if required
  • 1 x Full mouth x-ray – if required
  • Limited examination (diagnosis)
  • Local anaesthetic to numb the tooth during the procedure
  • Non-surgical tooth extraction (one tooth)
  • For the avoidance of doubt, if a tooth is assessed and requires a surgical extraction, this will not be included and the patient will be referred to a dental practitioner who is suitably qualified to perform that treatment.

Terms and conditions of treatment:  If the Fixed Price Dental treating dentist evaluates your tooth as a surgical extraction and not a basic extraction they will give you the choice to either stop the treatment and there will be no further cost to you or you can opt to have the tooth surgically removed by the Fixed Price Dental Dentist and pay for fixed price treatment for the surgical extraction (less your pre-paid booking fee).*The Fixed Price Dental basic tooth extraction appointment covers the following dental codes:  X-rays (022 x 2) and OPG (037), (311) Extraction of tooth, unless one or more of these items are deemed unsuitable for the patient, in the clinical opinion of the dentist. Fixed Priced approved Dentists must use only these codes for this treatment. If the Fixed Price dentist evaluates your tooth as an unsuitable extraction they will stop the treatment, *The Fixed Price Dentist reserves the right to charge a diagnosis fee should treatment not go ahead, this is limited to item numbers 013 (examination), 022 (x-ray) and 037 (OPG) and can not exceed the cost of $99.00 (including the prepaid booking fee).  

 

Surgical Extraction

Only the following treatments are included (as may be required)
  • 1 x periapical and 1 x Bitewing x-ray – if required
  • 1 x Full mouth x-ray – if required
  • Limited examination (diagnosis)
  • Local anaesthetic to numb the tooth during the procedure
  • Surgical tooth extraction (one tooth)
 
Terms and conditions of treatment:  If the Fixed Price Dental treating dentist evaluates your tooth as an unsuitable surgical extraction they will stop the treatment and there will be no further cost to you, you will be referred to a suitable specialist.

*The Fixed Price Dental Surgical extraction covers the following dental codes -examination (013), x-rays (022 x 2), OPG (037) & Surgical extraction of a tooth (324) unless one or more of these items are deemed unsuitable for the patient, in the clinical opinion of the dentist. Fixed Priced approved Dentists must use only these codes for this procedure.If the Fixed Price dentist evaluates your tooth as an unsuitable surgical extraction they will stop the treatment .

** The Fixed Price Dentist reserves the right to charge a diagnosis fee should treatment no go ahead this is limited to item numbers 013 (examination), 022 (x-ray) and 037 (OPG) and can not exceed the cost of $99.00 (including the prepaid booking fee). You will be referred to a suitable specialist for the surgical removal of your tooth.

 

 
 

Wisdom Tooth Removal

Only the following treatments are included (as may be required for initial consultation)
  • 1 x periapical and 1 x Bitewing x-ray – if required
  • 1 x Full mouth x-ray – if required
  • Limited examination (diagnosis)
Terms and conditions of treatment:  If the Fixed Price Dental treating dentist evaluates your tooth as an unsuitable wisdom tooth extraction they will stop the treatment and there will be no further cost to you, you will be referred to a suitable specialist for the surgical removal of your tooth.

Wisdom Tooth Extraction Terms and conditions of treatment:  *The Fixed Price Dental Wisdom tooth removal consultation covers the following dental codes -examination (013), x-rays (022 x 2) and OPG (037). Once the wisdom tooth is deemed appropriate for removal the following items numbers can be used in accordance with the Wisdom Tooth Removal fee. Surgical removal of a tooth or tooth fragment requiring both removal of bone and tooth division (324), The removal of bone or tooth division (322) unless one or more of these items are deemed unsuitable for the patient, in the clinical opinion of the dentist. Fixed Priced approved Dentists must use only these codes for this procedure. If the Fixed Price dentist evaluates your tooth as an unsuitable wisdom tooth extraction they will stop the treatment.

**The Fixed Price Dentist reserves the right to charge a diagnosis fee should treatment no go ahead this is limited to item numbers 013 (examination), 022 (x-ray) and 037 (OPG) and can not exceed the cost of $99.00 (including the prepaid booking fee). You will be referred to a suitable specialist for the surgical removal of your tooth.

 

Root Canal Therapy 

Root Canal Therapy Consultation- only the following treatments are included (as may be required)
  • Initial examination
  • 4 x Bitewing x-rays
  • 1 x Full mouth x-ray

Root Canal Therapy Treatment- only the following treatments are included (as may be required)

  • Removal of nerve
  • Cleaning of the nerve canal and each additional canal
  • Filling the nerve canal and each additional nerve canal
  • Irrigation or additional cleaning of the canal/s
  • Final restoration or preliminary restoration for a crown
Root Canal Therapy Terms and conditions of treatment:
*The Fixed Price Root canal therapy consultation appointment covers the following dental codes:  X-rays (022 x 2) and OPG (037), (013) Limited examination.
 ** The Fixed Price Dental Root Canal Treatment covers the following dental codes :Removal of the nerve (419), cleaning of there nerve canal and each additional canal (415 & 416-additional), Filling of the canal and each additional canal (417 & 418-additional), Additional irrigation and or dressing of the root canal per tooth (455), final or preliminary restoration for the tooth. Unless one or more of these items are deemed unsuitable for the patient, in the clinical opinion of the dentist. Fixed Priced approved dentists must use only these codes for the above mentioned treatment. 
If the Fixed Price Dental treating dentist evaluates your tooth as not being fit to have this treatment done the dentist will stop treatment and make the appropriate referral to a specialist. 
***The Fixed Price Dentist reserves the right to charge a diagnosis fee should treatment not go ahead this is limited to item numbers 013 (examination), 022 (x-ray) and 037 (OPG) and can not exceed the cost of $99.00 (including the prepaid booking fee)
 
 
 
 
 
 
 
 
 
 
 
 
Dental Crown

Only the following treatments are included (as may be required):

  • Limited examination (diagnosis)
  • 1or 2 bitewing -rays
  • Local anaesthetic to numb the tooth during the procedure
  • Preparation of the tooth for crown fitment
  • Second appointment to fit crown

Terms and conditions of treatment:  If the Fixed Price Dental treating dentist evaluates your tooth and the tooth is not fit to have this treatment done, no balance fee will be payable to the treating dentist or dental clinic.

*The Fixed Price Dental crown appointment covers the following dental codes:  (615- full crown) X-rays (022 x 2) and (013), unless one or more of these items are deemed unsuitable for the patient, in the clinical opinion of the dentist. Fixed Priced approved Dentists must use only these codes for this treatment.

 

Zoom in Clinic Teeth Whitening 

Only the following treatments are included (as may be required):

  • Limited examination (diagnosis)
  • Preparation of the teeth and gums for Zoom Teeth Whitening
  • 1hr in Chair Zoom Whitening

Terms and conditions of treatment:  If the Fixed Price Dental treating dentist evaluates your teeth is not fit to have this treatment done, no balance fee will be payable to the treating dentist or dental clinic.

*The Fixed Price Dental Zoom Whitening treatment appointment covers the following dental codes:  (118- multiplied by the number of teeth) and (013) unless one or more of these items are deemed unsuitable for the patient, in the clinical opinion of the dentist. Fixed Priced approved Dentists must use only these codes for this treatment.

 

Implant Consultation

Only the following treatments are included (as may be required)
  • Initial examination
  • 2 x Bitewing x-rays
  • 1 x Full mouth x-ray
Terms and conditions of treatment:  If the Fixed Price Dental treating dentist evaluates your bone and if the area is not fit to have this treatment done, no balance fee will be payable to the treating dentist or dental clinic.
*The Fixed Price Dental Implant consultation covers the following dental codes -examination (011, 013), x-rays (022 x 2) and OPG (037) unless one or more of these items are deemed unsuitable for the patient, in the clinical opinion of the dentist. Fixed Priced approved Dentists must use only these codes for this consultation.
 
 

Denture Consultation

Only the following treatments are included (as may be required)
  • Initial examination
  • 1 x Full mouth x-ray
Terms and conditions of treatment:  If the Fixed Price Dental treating dentist evaluates your suitability and wether it is viable to have this treatment done, if it’s not suitable no balance fee will be payable to the treating dentist or dental clinic.
*The Fixed Price Dental Denture consultation covers the following dental codes -examination (011, 013),  OPG (037) unless one or more of these items are deemed unsuitable for the patient, in the clinical opinion of the dentist. Fixed Priced approved Dentists must use only these codes for this consultation.
