Terms & Conditions (Dental Providers)

These Terms and Conditions apply to the following dental services provided by Fixed Price Dental approved Dentists

Services

We reserve the right to unilaterally amend these Terms and Conditions at any time. All amendments to these Terms and Conditions will be posted to the Fixed Price Dental website.

General

  • The fixed fee is for each standard fixed fee service or any part thereof.

  • If any of the included treatments for any particular services are not required the cost of the appointment will not be reduced.

  • The fixed fee will not be altered in relation to the number of teeth present in a patient.

  • Bitewing x-rays and Panoramic OPG (Full mouth) x-rays may not be required at every visit but may be recommended for all new patients as a baseline recording.

  • The patient’s (or parent/caregiver’s) consent will be sought before performing any additional treatment(s) beyond the initial Service. A separate appointment may be required to perform any additional treatment(s).

  • Any additional treatments will be made directly between your clinic and the patient.

  • If the recommended additional treatment that is not a part of the Fixed Price Dental scope of Services the patient will be referred to a dental practitioner who is suitably qualified to perform that treatment . That refereed dental practitioner will inform the patient of any fees which may be associated with that additional treatment.

  • For the avoidance of doubt, Fixed Price Dental is not liable for any loss or damage caused by any practitioner, the practitioner hold their own medical indemnity insurance and is liable for any work they have performed on patients.

  • Bulk-billed dental items are subject to the terms and conditions applying to the child dental benefits schedule. Please refer to the Australian Government Website for further information.

  • Set out below are the treatments included within each of the Services.

Fees and Cancellation Policy

If a patient cancels or FTA (fail to attend) a confirmed booking which there has been confirmed with a prepaid booking deposit online, you will forfeit your booking deposit.

If a patient is more than 20 minutes late for their appointment, then this is treated as a cancellation and the booking deposit with be forfeited if the booking was confirmed with a pre-paid booking deposit.

Payment Service

Our website includes a Payment Service, patients (* except for prearrangements with some integrated practice management software companies) must use the payment service to pay the treatment booking deposit (this booking deposit is deducted from the total treatment cost, and applies to the initial appointment, as additional treatments are booked directly with your clinic). The balance of the treatment cost (less the booking deposit will be paid by the patient directly to the Fixed Dental approved dental clinic on the day of treatment. The payment service may be provided by one or more third parties (the payment provider)

The payment of the booking deposit for the initial treatment is taken to be the patients consent to receive that treatment. The patients card details are processed and handled through the Payment Service. The patients payment details will be stored by the payment gateways and a secured token will be provided to Fixed Price Dental. The Payment Provider adheres to the standards set by the “Payment Card Industry Data Security Standard”.

Private Health Insurance

Patients with private health insurance that includes dental extras can use their private health insurance to make a claim on the dental treatments they receive from Fixed Price Dental approved Dentists

We have listed in our Terms and Conditions the Dental item codes for that can be claimed by the Fixed Price Dental partnering Dentists from the private health funds. No other item codes can be claimed other than what is listed.

Certain private health funds will pay more than the advertised treatment cost on the Fixed Price Dental website, in this instance the approved Fixed Price Dental clinic will note the amount of the overpayment as a credit on the patients clinic file and the patient can use this credit towards any future treatment (future treatment is booked directly between the patient and your dental clinic) .In some cases the private health fund will pay less than the advertised fee on the Fixed Price Dental website, in this instance the patient will need to make up the shortfall or (gap payment). The gap payment will be the difference between the amount the private health fund pays (once the allowed dental item codes are claimed) and the advertised cost of the Fixed Price Dental treatment (less any booking deposit that the patient has already paid, if this is the initial appointment)

*The exact amount that the private health fund pays on dental services depends on the level of the patients cover.

How We Provide the Services

We arrange for third party approved dental clinics to provide the treatments. If for any reason the providing dental clinics needs to defer or cancel a booking, then it is the responsibility of the providing dental clinic to arrange an alternative date/time for the booking that suits both the patient and the provider.

How We Communicate With You Regarding New Patients/Appointments

We will communicate with you via email. Your clinic will receive an email message (via the email address you provide us) notifying you the details of the booking and also the patients details. Clinics that are using Practice Management Software that is integrated with Fixed Price Dental will also receive a confirmation email of the new appointment, and the patients details. The appointment and patient details will populate your appointment schedule automatically if you are using integrated Practice Management Software.

If your Practice is using our “Side Bar” feature (for Practices not using integrated Practice Management Software, the patient details and appointment details must be entered into your system manually.

Your Obligations To The Patient

After the initial appointment booking is made and confirmed by the patient, you will receive an email from Fixed Price Dental alerting you of this appointment , any further communication with the patient is to be made directly between your clinic and the patient.

Our Fixed Price Treatments and Item Codes Schedule

The Fixed Price Dental fee schedule is to be used when a Fixed Price Dental referred patients attends their initial appointment at your clinic and when they return to your clinic for further treatments. As the patient will book directly with your clinic for future treatment it must be noted in their patient file that they are a Fixed Price Dental referred patient so that they receive the Fixed Price Dental advertised fees.

The Fee schedule and codes for each treatment are there as a guide to the codes that we allow to be charged/claimed when the patient is using their private health insurance.

As most of our Fixed Price Dental treatments are advertised as a complete package price, the patient must not be charged more than our advertised treatment package cost, even if the sum of the codes used is more or extra codes etc were used in the treatment.

Check Up and Clean

Only the following treatments are included (as may be required)

  • Initial examination or checkup

  • 2 x Bitewing x-rays

  • 1 x Full mouth x-ray

  • Removal of calculus

  • Plaque/stain removal

  • Topical application of remineralising and/or cariostatic agent

*The Fixed Price Dental check-up & clean treatment covers the following dental codes -examination (011, 012), scale and clean (114) or removal of plaque (111), fluoride (121), x-rays (022 x 2) and OPG (037) unless one or more of these items are deemed unsuitable for the patient, in the clinical opinion of the dentist. Fixed Priced approved Dentists must use only these codes for this treatment.

Comprehensive Dental Examination

Only the following treatments are included (as may be required)

  • Initial examination or check-up

  • 2 x Bitewing x-rays

  • 1 x Full mouth x-ray

*The Fixed Price Dental Comprehensive Dental Examination treatment covers the following dental codes -examination (014),(022 x 2) and OPG (037) unless one or more of these items are deemed unsuitable for the patient, in the clinical opinion of the dentist. Fixed Priced approved Dentists must use only these codes for this treatment.

Fillings

Only the following treatments are included (as may be required):

  • 1 x Bitewing x-ray or 1 x periapical x-ray

  • 1 x Full mouth x-ray – if required

  • Diagnosis of the issue

  • Local anaesthetic to numb the tooth during the procedure.

  • A tooth-coloured direct restoration involving up to four surfaces of a single tooth (commonly known as a filling).

  • For the avoidance of doubt, teeth requiring a filling involving more than 4 tooth surfaces will not be included and the patient will be referred to another dental practitioner.

*The Fixed Price Dental filling treatment covers the following dental codes: x-rays (022 x 2) and OPG (037), (531) – one surface filling OR (532) – two surface filling OR (533) – three surface filling or (534) – four surface filling , unless one or more of these items are deemed unsuitable for the patient, in the clinical opinion of the dentist. Fixed Priced approved Dentists must use only these codes for this treatment.

Emergency Appointment - Pain Relief

Only the following treatments are included (as may be required):

  • Limited examination (diagnosis)

  • 1 x Bitewing x-ray or 1 x periapical x-ray

  • 1 x Full mouth x-ray) – if required

  • Diagnosis of the cause of the pain

  • Local aesthetic to numb the tooth during the procedure

Depending on the cause of the pain one of the following options may also be included:

  1. First stage Root Canal Therapy

  2. Simple extraction of the tooth

  3. Tooth Desensitising application

  4. Deep cleaning around a tooth to treat an acute gum infection

  5. A temporary filling (for cases where this is the most appropriate solution to relieve pain until a more longterm solution can be performed)

*The Fixed Price Dental Pain relief- emergency appointment covers the following dental codes: (013) Oral Examination- Limited, X-rays (022 x 2) and OPG (037), (419) Extirpation, (311) Basic Extraction, (572) Temporary filling, unless one or more of these items are deemed unsuitable for the patient, in the clinical opinion of the dentist. Fixed Priced approved Dentists must use only these codes for this treatment.

Basic (Simple) Tooth Extraction

Only the following treatments are included (as may be required):

  • 1 x periapical and 1 x Bitewing x-ray – if required

  • 1 x Full mouth x-ray – if required

  • Limited examination (diagnosis)

  • Local anaesthetic to numb the tooth during the procedure

  • Non-surgical tooth extraction (one tooth)

*The Fixed Price Dental basic (simple) tooth extraction appointment covers the following dental codes: X-rays (022 x 2) and OPG (037), Oral Examination - Limited (013), Extraction of tooth (311), unless one or more of these items are deemed unsuitable for the patient, in the clinical opinion of the dentist. Fixed Priced approved Dentists must use only these codes for this treatment.

Dental Crown

Only the following treatments are included (as may be required):

  • Limited examination (diagnosis)

  • 1 or 2 bitewing - rays

  • Local anaesthetic to numb the tooth during the procedure

  • Preparation of the tooth for crown fitment

  • Second appointment to fit crown

*The Fixed Price Dental crown appointment covers the following dental codes: (613 - full crown) X-rays (022 x 2) and Oral Examination - Limited (013), unless one or more of these items are deemed unsuitable for the patient, in the clinical opinion of the dentist. Fixed Priced approved Dentists must use only these codes for this treatment.

Dental Veneers

Only the following treatments are included (as may be required):

  • Limited examination (diagnosis)

  • 1 or 2 bitewing - rays

  • Local anaesthetic to numb the tooth during the procedure

  • Preparation of the tooth for Veneer fitment

  • Second appointment to fit Porcelain Veneer

*The Fixed Price Dental Veneer appointment covers the following dental codes: (556) – Porcelain Veneer per tooth, X-rays (022 x 2) and Oral Examination - Limited (013), (526) Composite Veneer per tooth unless any of these items are deemed unsuitable for the patient, in the clinical opinion of the dentist. Fixed Priced approved Dentists must use only these codes for this treatment.

Surgical Extraction (inc. Wisdom teeth)

Only the following treatments are included (as may be required)

  • 1 x periapical and 1 x Bitewing x-ray – if required

  • 1 x Full mouth x-ray – if required

  • Limited examination (diagnosis)

  • Local anaesthetic to numb the tooth during the procedure

  • Surgical tooth extraction (one tooth)

*The Fixed Price Dental Surgical extraction covers the following dental codes -examination (013), x-rays (022 x 2), OPG (037) & Surgical extraction of a tooth (322), (323), (324) unless one or more of these items are deemed unsuitable for the patient, in the clinical opinion of the dentist. Fixed Priced approved Dentists must use only these codes for this procedure.

Root Canal Therapy

Only the following treatments are included (as may be required)

  • Initial examination

  • 4 x Bitewing x-rays

  • 1 x Full mouth x-ray

  • Removal of nerve

  • Cleaning of the nerve canal and each additional canal

  • Filling the nerve canal and each additional nerve canal

  • Irrigation or additional cleaning of the canal/s

  • Final restoration

*The Fixed Price Root canal therapy appointment covers the following dental codes: X-rays (022 x 2), OPG (037), (013) Limited examination and Front Tooth (415), (417) Premolar Tooth (415), (416), (417), (418). Molar Tooth (415), (416), (416), (417), (418), (418) Additional irrigation and or dressing of the root canal per tooth (455), final or preliminary restoration for the tooth.

Unless one or more of these items are deemed unsuitable for the patient, in the clinical opinion of the dentist. Fixed Priced approved dentists must use only these codes for the above mentioned treatment.
